[NETFILTER]: nf_conntrack_extend: warn on confirmed conntracks

Browse Source 
New extensions may only be added to unconfirmed conntracks to avoid races
when reallocating the storage.

Also change NF_CT_ASSERT to use WARN_ON to get backtraces.
