[ARM] Remove MODE_(SVC|IRQ|FIQ|USR) and DEFAULT_FIQ

Browse Source 
DEFAULT_FIQ was entirely unused.  MODE_* are just redefinitions
of *_MODE.  Use *_MODE instead.
