CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

SPARSE_IRQ is required for single zImage support.
With this enabled, we can delete <mach/irqs.h>. This requires removing
one unnecessary include of that file, and hard-coding the PCIe IRQ into
the PCIe driver. This is a hack that will be dealt with as part of
converting the PCIe driver into a true DT-supporting driver.

Nothing outside mach-tegra uses this file, so there's no need for it to
be in <mach/>.
Since uncompress.h and debug-macro.S remain in include/mach, they need
to include "../../iomap.h" becaue of this change. uncompress.h will soon
be deleted in later multi-platform/single-zImage patches. debug-macro.S
will need to continue to include this header using an explicit relative
path, to avoid duplicating the physical->virtual address mapping that
iomap.h dictates.

This is the 2nd part of mach/io.h removals. This series removes io.h on
platforms with PCI by creating a fixed virtual I/O mapping and a common
__io() macro.
This version has changed a bit to accommodate Tegra converting its PCIe
host to a platform driver. Now the virtual space is only reserved during
early boot before .map_io() is called. The mapping is not created until
calling pci_ioremap_io which can be done at any point after vmalloc is
initialized.
I've gone back to fixed 64K windows for each PCI bus. This allows
removing all the i/o resource setup from the individually platforms and
placing it within the common ARM PCI code.
I've only tested versatilepb under qemu (with the model hacked up to
actually enable i/o space), so any testing is appreciated. iop3xx and
mv78xx0 have some risk of breaking as the PCI bus addresses are moved
to 0 from matching the cpu host bus addesss.

Most PCI implementations use the standard PCI swizzle function, which
handles the well defined behaviour of PCI-to-PCI bridges which can be
found on cards (eg, four port ethernet cards.)
Rather than having almost every platform specify the standard swizzle
function, make this the default when no swizzle function is supplied.
Therefore, a swizzle function only needs to be provided when there is
something exceptional which needs to be handled.
This gets rid of the swizzle initializer from 47 files, and leaves us
with just two platforms specifying a swizzle function: ARM Integrator
and Chalice CATS.

In previous patch (arm/tegra: add timeout to PCIe PLL lock detection loop)
tegra_pcie_enable_controller() function type has been changed from
void to int, but the last return statement wasn't converted, so
function returns undefined value. Fix it.
Also while at it, address couple of minor concerns raised by reviewers:
use usleep_range for delay, and lower the value of timeout to 300ms
to be consistent with Nvidia Vibrante kernel.

Tegra PCIe driver waits for PLL to lock using busy loop.
If PLL fails to lock for some reason, this leads to silent lockup
while booting (as PCIe code is not modular).
Fix by adding timeout, so if PLL doesn't lock in a couple
of seconds, just PCIe driver fails and machine continues to boot.

Convert from pci_scan_bus() to pci_scan_root_bus() and remove root bus

Convert PCIBIOS_MIN_IO and PCIBIOS_MIN_MEM to variables to allow
multi-platform builds. This also removes the requirement for a platform to
have a mach/hardware.h.
The default values for i/o and mem are 0x1000 and 0x01000000, respectively.
Per Arnd Bergmann, other values are likely to be incorrect, but this commit
does not try to address that issue.
